Participant Waiver

LONE STAR TRACK & FIELD CLUB
PARTICIPANT WAIVER, RELEASE OF LIABILITY, AND ASSUMPTION OF RISK

This Waiver and Release of Liability ("Agreement") is entered into by the parent or legal guardian ("Signer") of the minor athlete ("Participant") identified below, in connection with the Participant's enrollment in Lone Star TFC training programs for the current 8-week block.

1. ASSUMPTION OF RISK. I acknowledge that participation in track and field training, including but not limited to sprinting, jumping, throwing, strength training, and conditioning activities, involves inherent risks of physical injury. I voluntarily assume all such risks on behalf of the Participant.

2. RELEASE OF LIABILITY. To the fullest extent permitted by law, I hereby release, waive, discharge, and covenant not to sue Lone Star TFC, its coaches, staff, volunteers, and facility partners from any and all claims, demands, or causes of action arising out of or related to the Participant's participation in any Lone Star TFC program or activity, including claims arising from negligence.

3. MEDICAL AUTHORIZATION. I authorize Lone Star TFC staff to obtain emergency medical treatment for the Participant if I cannot be reached. I represent that the Participant is physically fit to participate and that I have disclosed any known medical conditions or limitations.

4. PHOTO/VIDEO RELEASE. I grant Lone Star TFC permission to use photographs and video recordings of the Participant for promotional and educational purposes, including on the club's website and social media channels.

5. CODE OF CONDUCT. I agree that the Participant will adhere to all Lone Star TFC rules and codes of conduct. Lone Star TFC reserves the right to dismiss any participant whose behavior is deemed detrimental to the program.

6. BLOCK-SPECIFIC AGREEMENT. This waiver applies to the specific 8-week training block identified below and must be re-signed for each subsequent block.

7. GOVERNING LAW. This Agreement shall be governed by the laws of the State of Texas.

By signing below, I confirm that I have read, understood, and agree to the terms of this Agreement.
